At the bottom of this page, you shall find my CLIENT INTERVIEW FORM with an attached Fee Agreement. Click on the appropriate link to download that form. It is in Microsoft Word format. If you can, you may print it and fill it out. Complete the client interview form to the best of your ability; and review and sign as is necessary on the agreement. Also, if you do not have copies of your last two years' taxes and have Adobe Acrobat Reader, you should also click on, download, print and complete the IRS TAX TRANSCRIPT REQUEST FORM. Finally, there is a SOCIAL SECURITY CARD ORDER FORM. When you go to Court hearing on your case, you must bring with you: (1) a photo ID, generally your driver's license; and (2) proof of your Social Security number, generally your Social Security card.

ONLINE: You may retain me online to the extent that you may pay me online and/or e-mail me some of the requested information online. Retain me online by making payment of your retainer to me via http://www.paypal.com. You or a third party on your behalf who is making payment must have a PayPal account, which is somewhat simple to set up, and you may pay through a VISA or Mastercard through PayPal. If you wish to pay with VISA or Mastercard without going through PayPal, simply contact my office and we can perform the transaction over the telephone. Direct your payment to jk_mitchell@sbcglobal.net, my registered e-mail there. Also, you may mail the completed client interview form to my office or e-mail it to jkm@oklahomabankruptcyattorney.net. Unless you wish to scan, create, upload and e-mail me the bulleted items listed below in the "By Mail:..." section, you must mail or deliver to me those items separately.
BY MAIL: Retain me by mail and save some driving and time. Mail the completed client interview form and agreement to me along with a check or money order in the amount of at least what is required to file for bankruptcy (See applicable pricing table in fee agreement on the front page of this website, http://www.oklahomabankruptcyattorney.net). Make your payment to "Joel K. Mitchell, Client Trust Acct." If you pay less than the full retainer amount, any remainder is due within the sooner of 30 days from filing or by the Meeting of the Creditors court date scheduled in your case (approximately one month from filing). In addition to enclosing payment, the interview form and signed agreement in the mail, or bringing those with you to your appointment, make sure you produce copies of the following items for me which are necessary to prepare your bankruptcy:
- TAX RETURNS, both federal and state, for the last 2 tax years (if you don't have these documents in your possession, download, print, complete and mail the attached Adobe Acrobat file with the IRS Tax Transcript Request form from this webpage; and if you have not yet filed the most recent tax year, then all W-2's and 1099's for that year);
- PAYSTUBS for the last 6 months from any employer you worked for during that time (if you didn't save them, you will need to obtain a summary printout from the employer);
- OTHER INCOME DOCUMENTATION, including that relating to a business or to any child support, alimony, pension income, social security, disability or DHS benefits;
- TITLES to all automobiles, motorcycles, boats, engines, trailers, etc., on which your name appears;
- MORTGAGES AND/OR DEEDS containing the legal description for any real property on which your name appears;
- ACCOUNT STATEMENTS, covering the last 3 months, for any checking, savings, investment, 401(k), or retirement account on which you are named;
- EXPENSE DOCUMENTATION covering the last 3 months of any utility bill, car payment, house payment, childcare or other expense you regularly make, and also any receipts you may have saved showing food and gasoline purchases (Much of this information will appear in your bank account statements, but do your best to round up what you can);
- DEBT STATEMENTS, whether recent or not, for any debts which Client seeks to discharge, credit card, medical, etc. (Of course, many will appear on your credit report too).
If you wish to conduct business by mail, you may do so by gathering the above-stated documentation and mailing it with your payment in a big envelope or small box (e.g., Priority mail) from your local post office to either of my offices but, preferably, to my post office box at the following address:

Client Interview Form and Agreement
I need the information in the client interview form to prepare your bankruptcy paperwork. I need you to read the agreement at the end so that you will understand both your and my responsibilities. Clients retaining me by mail need to complete this client interview form and sign the agreement and other documents, where indicated, in order for me to represent them in a Chapter 7 bankruptcy case. Clients who intend to come in-person for an appointment or consultation may download, fill out and sign the documents prior to meeting me in person and bring those documents within them, or we can complete those documents together at my office.

Social Security Card Request Form
Use this to order a new or a replacement Social Security Card. The form gives the website address and a 1-800 number to call for further instructions. You must bring your Social Security card with you to Court when you attend the meeting of the creditors.

List of Pre-Bankruptcy Credit Counseling Agencies
This is the Court-approved list of pre-bankruptcy credit counseling agencies for persons living in and wishing to file bankruptcy in the State of Oklahoma. You can use any of the agencies on this list. You can do this counseling before or after you retain me, but you must do it no more than 5 months before and no more than 30 days after you do retain me and sign my fee agreement [My thinking here is that the certificate the agency issues is only valid for 6 months, and my agreement requires you to get this done within 30 days of meeting me so as to not delay your bankrutpcy case].
